One of the most important of these signals is leptin—and supporting healthy leptin activity is being shown to offer a new way to support joint comfort and mobility.

Aching knees, stiff hips, and painful hands can gradually interfere with walking, exercise, sleep, work, and ordinary daily activities. These problems become especially common as we grow older and especially among people who are overweight or obese. Women may face additional challenges during and after menopause, when declining estrogen, changes in fat distribution, and gradual muscle loss can affect both metabolic and joint health.

Most people understand that carrying excess weight places more mechanical pressure on the knees, hips, feet, and lower back. But body fat can also affect the joints through a less visible pathway: metabolic communication.

Fat tissue releases chemical messengers called adipokines. These hormone-like substances travel through the bloodstream and influence appetite, insulin sensitivity, inflammation, immune activity, bone, cartilage, and the lubricating fluid inside our joints.
Leptin is one of the most important adipokines involved in this fat-to-joint conversation.

A Metabolic Approach to Joint Support

This emerging connection between body fat, leptin, and joint health is another reason I selected TruLeptin™ Modulator for the Truth for Health store. Instead of focusing only on the mechanical pressure excess weight places on the joints, TruLeptin™ Modulator supports the metabolic side of joint comfort.

Its key ingredient, ORALVISC®, is a proprietary complex of hyaluronic acid and other glycosaminoglycans. These compounds occur naturally in cartilage, connective tissue, and synovial fluid—the protective, lubricating fluid inside movable joints. For over a decade, ORALVISC has been clinically studied for its effects on serum and synovial-fluid leptin, inflammatory signaling, joint comfort, and physical function.

For older adults, menopausal women, hypogonadal men, and in general anyone carrying excess body fat, this approach is especially relevant. Anyone with these medical problems can experience both increased mechanical stress on their joints and also experience changes in adipokines released by fat tissue. Pain management approaches need to include strategies to improve healthy leptin activity within the adipose–brain–joint–metabolic network, which in turn reduces inflammatory pain signals and reduces pain that can help facilitate a more active lifestyle to support healthy fat loss.

Joint problems increase with age

Osteoarthritis, or OA, is the most common type of arthritis and affects approximately 33 million adults in the United States. Although it becomes more common after age 45, OA is not simply an inevitable or “normal” consequence of aging.

In June 2026, a national analysis estimated that OA affected approximately 13.6% of U.S. adults. Prevalence rose to 31.6% among adults age 65 and older. More than 80% of adults with OA were either overweight or obese, and the prevalence of OA increased progressively across higher body-mass-index categories.

Osteoarthritis can affect the whole joint—not only cartilage, but also the bone beneath the cartilage, synovial lining, ligaments, muscles, and sensory nerves. This helps explain why people can experience stiffness, swelling, reduced range of motion, instability, and pain even when an X-ray does not appear dramatically abnormal.

The older “wear-and-tear” description therefore tells only part of the story. Joint problems can reflect several interacting factors:

Mechanical loading and altered joint alignment.

Previous injuries or repetitive stress.

Age-related changes in tissue repair.

Loss of muscle strength and joint stability.

Insulin resistance and metabolic dysfunction.

Adipokine and immune signaling.

Genetic susceptibility.

Hormonal changes associated with menopause.

Fat is an endocrine organ

Most people think of body fat as stored energy. In reality, adipose tissue is also an active endocrine organ, meaning that it produces chemical messengers that influence tissues throughout the body.

Healthy adipose tissue stores energy safely and communicates appropriately with the brain, liver, muscles, pancreas, immune system, and joints. When fat cells enlarge—especially within visceral fat surrounding the abdominal organs—the tissue can become dysfunctional and insulin-resistant. It may then release more fatty acids, inflammatory mediators, and unfavorable adipokine signals.

These signals include leptin, adiponectin, resistin, chemerin, and other regulatory proteins. Together, they influence hunger, energy use, glucose metabolism, blood vessels, inflammation, bone remodeling, and cartilage biology, with leptin being the most significant.

Leptin’s Role

Fat cells release leptin largely in proportion to the amount of energy stored as body fat. Leptin travels through the bloodstream to the hypothalamus, a region of the brain that helps regulate hunger, energy expenditure, body temperature, reproduction, and other essential functions.

When this signaling system works properly, leptin gives the brain information about long-term energy availability. Rising leptin generally tells the brain that adequate energy is stored, helping to restrain hunger and support normal energy use. When leptin falls during fasting, calorie restriction, or weight loss, the brain receives a signal that stored energy has declined. Hunger may increase, while the body becomes more economical with its energy.

Leptin is not a “bad hormone.” We do not want to eliminate it, and at the same time, we need to avoid driving it too high OR too low. The goal is to have appropriate leptin production combined with appropriate responses to that signal.

But in obesity, leptin levels commonly rise to high levels but the brain does not respond adequately. Researchers call this pattern leptin resistance. It resembles a communication system in which the sender keeps raising its voice, but the receiver is “deaf” to the signal and no longer responds normally. Elevated leptin in this situation coexists with persistent hunger, impaired energy regulation, insulin resistance, and chronic low-grade inflammation. That in turn makes fat loss more and more difficult. It also makes movement, exercise in particular, more painful, and then people become more sedentary, creating a vicious cycle of pain leads to inactivity leads to gain in fat, more pain signals, more pain, and less physical movement.

Body fat “talks” to joints!

Leptin does not communicate only with the brain. It also enters the actual environment around our joints and interacts with several types of cells and tissues:

Chondrocytes, the cells that produce and maintain cartilage.

Synoviocytes, the cells lining the joint capsule and helping produce synovial fluid.

Osteoblasts and osteoclasts, which build and remodel bone.

Immune cells, which regulate inflammatory responses.

Pain pathways, which influence how the nervous system senses joint stress or injury.

Researchers have detected leptin in synovial fluid. Higher blood and synovial-fluid leptin levels have been associated with body-fat mass, joint symptoms, and osteoarthritis progression in several studies.

Laboratory research has shown that excessive leptin signaling activates inflammatory pathways within cartilage and other joint tissues. These pathways can increase cytokines—chemical messengers that regulate inflammation—and enzymes that participate in cartilage-matrix breakdown.

This metabolic connection helps explain why obesity can affect joints well beyond just weight-bearing ones like knees and hips. Hands can also be affected as well as other joints. Excess body weight does not mechanically load the hands in the same way it loads the knees, yet obesity also correlates with arthritis pain the hands.

Healthy Leptin Activity Matters

Supporting healthy leptin activity means more than changing one laboratory value. It means supporting appropriate communication among adipose tissue, the brain, metabolic organs, immune cells, and joints.

TruLeptin™ Modulator: Support for Fat-to-Joint Signaling

TruLeptin™ Modulator was specifically designed based on its ORALVISC complex to support healthier signaling within the fat-to-joint pathway via effects on leptin in both the circulation and synovial fluid.

This is important because synovial fluid does more than lubricate the joint. It surrounds cartilage and reflects part of the biochemical activity occurring inside the joint. Leptin, cytokines, chemokines, and pain-related mediators within this fluid influence how the joint responds to both mechanical and metabolic stress.

TruLeptin™ Modulator is formulated to support healthy blood serum and synovial-fluid leptin levels, which in turn helps promote a more balanced joint signaling and nutrient environment. TruLeptin™ Modulator as a daily supplement adds another nutritional strategy for supporting comfort and function as we get older or put on a few extra pounds.

The Bottom Line

Body fat and joints constantly exchange biological messages. Mechanical loading still matters, but adipose tissue also communicates through leptin and other adipokines that can influence appetite, metabolism, inflammation, cartilage, synovial tissue, bone, and pain pathways.

TruLeptin™ Modulator brings this metabolic perspective to joint support. Its ORALVISC complex combines hyaluronic acid with other naturally occurring glycosaminoglycans to support synovial health, healthy leptin activity, joint comfort, and physical function.

In a randomized, placebo-controlled study of adults with obesity and knee joint problems, daily ORALVISC supplementation was associated with lower serum and synovial-fluid leptin, improvements in inflammatory and pain-related mediators, and significant improvements in joint comfort and function. These findings support a broader approach to joint care—one that recognizes that body fat affects the joints through both mechanical pressure and metabolic communication.

For people who carry excess weight, experience age- or menopause-related metabolic changes, or find that joint discomfort interferes with staying active, TruLeptin™ Modulator adds valuable nutritional support. By helping support a healthier adipose–joint environment, it may make comfortable movement more achievable—and comfortable movement is essential for maintaining strength, managing weight, protecting cardiovascular and metabolic health, and preserving independence.
We need to support the signal, support the joint and keep moving!

Menopause changes the equation for women

Midlife women often notice changes in body composition even when the number on the scale changes very little. Fat may shift toward the abdomen and around abdominal and pelvic viscera (“visceral fat”), even though muscle mass and strength gradually decline. Increase in visceral fat and loss of lean muscle is a high risk combination that in turn reduces insulin sensitivity and alter adipokine signaling.

Estrogen also interacts with cartilage, bone, muscle, immune pathways, and pain processing. A recent review found a relationship between menopause and greater OA severity, although human studies have not consistently demonstrated that menopause alone causes measurable cartilage loss.

So this changing balance in hormones and body composition presents a serious challenge for menopausal women: less muscle to absorb force and stabilize the joints, more metabolically active abdominal fat, changes in bone and cartilage biology, and altered pain processing. I am not saying that every single symptom of menopausal joint pain results from estrogen loss or increased leptin, but I am saying we have to take these additional critical factors into account in order to design individualized, integrated and comprehensive approaches to overall pain management, especially joint pain that limits activity and impairs our independence as we get older.

TruLeptin™ Modulator may matter even more at midlife, since it adds targeted support for this changing metabolic-hormonal balance. It helps support healthy leptin activity and supplying a clinically studied glycosaminoglycan complex, it helps support the adipose–joint connection, joint comfort, and physical function.

Maintaining comfortable movement definitely makes helps us walk more, exercise, preserve muscle, manage weight, and remain independent.

Break The Pain–Inactivity Cycle

Joint discomfort can initiate a self-reinforcing cycle:

Pain makes walking, climbing stairs, and exercise more difficult.

Reduced activity weakens the muscles that support and stabilize the joints.

Lower activity reduces daily energy expenditure and may promote visceral-fat gain.

Additional adipose tissue increases both mechanical loading and unfavorable metabolic signaling.

Greater stiffness and discomfort make activity even more difficult.

Supporting joint comfort can help create an opportunity to interrupt this cycle which is why I often recommend TruLeptin™ Modulator to many of my patients caught in this vicious cycle.

Build strength to protect joints

Strong muscles absorb force, stabilize joints, improve balance, and make everyday movement easier. If medically appropriate, aim for progressive resistance exercise at least twice each week. If you have significant pain, instability, or mobility limitations you may benefit from working with a physical therapist or certified exercise professional.

Useful options can include sit-to-stand practice, supported squats to a comfortable depth, step-ups, hip strengthening, resistance bands, weight machines, water exercise, and upper-body training. A safe program should challenge the muscles without repeatedly aggravating the joint.

Keep moving at your own level

Walking, cycling, swimming, aquatic exercise, tai chi, and light dancing can provide cardiovascular and mobility benefits with relatively low joint stress. Activity can be divided into multiple short sessions until you build up to longer workouts.

We know that these joint-friendly physical activities can reduce arthritis pain, improve function and mood, delay disability, and as well as help manage many other chronic conditions.

Tip: A useful principle is to begin with low intensity and increase slowly. Start at an exercise level below the level that triggers a flare-up of your pain and then increase very gradually. Mild, temporary discomfort does not always indicate joint damage. Make sure you have adequate rest and recovery to help you build strength, and exercise tolerance. If you experience any sharp pain, marked swelling, instability, or symptoms that worsen substantially after exercise, however, that can mean you did too much too fast and that you need to back off the intensity, amount of weight, number of repetitions or sets, or length of time for some of your exercises. Working with a physical therapist or certified exercise professional can help you avoid more flare-ups or injury. Note: if the new symptoms do not resolve after a few days of rest, may need to seek medical attention.

Improve body composition

For people who are overweight or obese and who have knee or hip arthritis pain, reducing excess body fat can decrease mechanical loading and improve symptoms. Loss of body fat combined with exercise produces better pain relief than relying on fat loss alone. You really need to focus on protecting your muscle mass when trying to lose fat. The goal is not “weight loss” as I wrote in my earlier Health Tip on this point. It is FAT loss and preserving muscle, which may not mean a lot of actual pounds lost. The goal is better measured by loss of INCHES, not pounds on the scale.

Adequate protein, progressive resistance exercise, and a moderate calorie deficit help preserve the tissue that stabilizes joints and maintains function. Crash dieting, and rapid “weight loss” with GLP-1 drugs cause rapid loss of scale weight but can cost you 25% or more of critical lean muscle and bone also lost along with fat! For people over 40, losing muscle and bone is harder and harder to gain back, and means more of your body composition is fat going forward. That’s a terrible trade for long-term joint and metabolic health!

Choose nutrition that reduces pain and maintains healthy weight

Build meals around high-quality proteins, vegetables, beans, intact whole grains, nuts, seeds, whole fruit, oily cold-water fish, and healthy fats such butter, tallow from grass-fed beef, or extra-virgin olive oil, coconut oil or avocado oil. Reduce ultra-processed foods, refined carbohydrates, foods with added sugars and sweetened beverages (including artificial sweeteners), and eliminate or reduce alcohol.

Eat real foods that support optimal balance of protein, fat, complex carbs for energy and fiber and this combination improves satiety, glucose regulation, cardiovascular health, and helps you gradually lose the excess fat. You will also find that weight management easier is without extreme restriction and cravings are reduced because you are giving your body what it needs.

Protect sleep and recovery

Poor sleep duration and quality definitely increases pain by many different pathways, including increased sensitivity to pain stimuli, loss of energy and reduced activity, impaired glucose regulation, increase in cortisol release (our stress hormone), and making appetite harder to manage. Aim for a consistent sleep schedule, and practice good sleep hygiene (see my earlier Health Tip in the archive on ways to improve your sleep!). Work on eliminating all the factors in your environment that repeatedly disrupt sleep.

You really need to see a Sleep Medicine medical professional if you have persistent loud snoring, your sleeping partner witnesses you stop during the night and then “gasp” awake, or you have morning headaches, or excessive daytime sleepiness warrant. Obstructive sleep apnea increases significantly in older people with heavy central body fat gain, diabetes, hypertension, or other medical conditions that disrupt sleep. Sleep apnea can cause sudden herat attacks or strokes, and needs to be treated properly to restore healthy sleep quality and reduce your risk of life-threatening events.

Treat the whole joint problem

Leptin support should complement—not replace—appropriate joint evaluation and treatment. A comprehensive plan may include education, exercise, weight management, physical therapy, braces or assistive devices, topical pain relievers or oral pain medications when medically suitable, injections, and surgery for advanced joint damage. I always suggest my patients to seek professional guidance that emphasizes individualized, integrated, multidisciplinary care.

Do not assume that every painful joint represents osteoarthritis. Prompt medical assessment is particularly important for a hot, red, markedly swollen joint; sudden severe pain; fever along with joint pain symptoms; inability to bear weight after an injury; new weakness or numbness; or calf swelling accompanied by chest pain or shortness of breath.

For more research information and references, please review my original health tip on
